If you intend to undertake your CPL/IR in Italia, you will never need a UK CAA issued Class 1 Medical, as you will be applying to the Italian Aviation Authority (not sure of the official name in Italiano) for your license, so you will need the Italian Class 1 there.
I recommend that you contact the Italian Aviation Authority before you enrol on our Distance Learning course and make sure they will accept your plan to take the 14 JAA Exams in the UK and then do the ME/CPL/IR in Italia. I don't think they will object, but better safe than sorry.
Here are our official requirements regarding Class 1 Medicals, as published in our Training Manuals. What we publish in these manuals for each course is approved by the UK CAA and is what they hold us accountable to enforce:
Modular Ground School students, whether they take the Classroom Based Residential (CBR) or the Distance Learning (DL) ATPL Theory course, do not have to have a JAA Class 1 Medical to enrol or complete the course. It is advisable that they have a Class 1, but not mandatory.
Waypoint Modular (ME/CPL/IR/MCC) students must have a JAA Class 1 Medical to enrol on the course. They should convert any non-UK JAA Class 1 Medical to a UK CAA issued JAA Class 1 Medical before the IR Skills Test so that we may apply to the UK CAA for their CPL/IR, which can only be issued if their Class 1 was also issued by the UK CAA.
APP First Officer students must have a JAA Class 1 Medical before commencing the course and if it was not issued by the UK CAA, must be converted prior to the IR Skills Test.
